Sandra Regains Independence!
Sandra D. admitted to Crystal Care Center of Ashland after being hospitalized for a hip surgery. Prior to her surgery, Sandy was independent at home. She admitted with weight bearing restrictions on her left lower extremity. Her functional mobility and self-care status at her physical therapy and occupation therapy evaluations indicated poor standing balance, maximum assistance for bed mobility, moderate assistance for functional transfers, inability to ambulate, and required assistance for ADL activities. Upon discharge, Sandy was able to independently walk 75 feet with a walker, perform transfers, bed mobility, manage self-care, and ADL tasks. She was also able to ascend/descend 3 steps with handrails and stand-by assistance. Sandy was able to return home safely with family support.
