Jeannette R. Returns Home!
Jeanette admitted to Crystal Care of Ashland after a lengthy hospital stay resulting in decreased strength, endurance, balance, and inability to care for herself. She also had an altered diet and PEG tube for nutrition for a swallowing disorder. Prior to her hospitalization, Jeanette was independent at home. When she admitted she could not walk, required moderate assistance for bed mobility, maximum assistance for functional transfers, poor standing balance, moderate assistance for ADL’s and self-care, and was on a puree diet with additional nutritional support through the PEG tube. After extensive therapy services, Jeanette was able to safely discharge home with family support. On the day of discharge, Jeanette was able to walk 250 feet with FWW with supervision, ascend/descend 6 steps with bilateral handrails with CGA, perform bed mobility, transfers, and ADL’s/self-care with supervision. She was able to eat a regular diet and the PEG tube was able to be removed successfully.
